Please mail this application postmarked by June 1, 2007 via USPS standard mail to the address listed below.  Do not send registered, certified, or express mail.  Emailed entries will not be accepted.

 

Include the following in your scholarship application packet:

·         This application form

·         Original photograph or artwork, which represents your Portrait of America.  Please refer to the web site for more information about this topic if necessary.

·         Please keep the master copy of your photos and artwork. For the contest, please just submit a duplicate photo or photo of your artwork.  Please be sure to submit a photo large enough for us to easily review.  4x6, 5x7, and 8x10 are all acceptable.  Finalists may be asked for additional photos and/or electronic copies of the photos (ex: .bmp, .pdf, .tif, or similar). 

 

Eligibility:

·         Must be a U.S. citizen

·         Must be enrolled in high school for the 2006-2007 school year: public, private, or home school are all acceptable
